Job Description:
Crossbridge Hospice seeks a compassionate and dedicated Medical

illnesses and their families. This role involves collaboration with
a multidisciplinary team to enhance the quality of care and
ensuring that the psychosocial needs of patients and their families

complexities associated with end-of-life care. The successful
candidate will be responsible for a wide range of services
including assessments, treatment planning, and crisis
intervention.
