A COLLECTIVE BARGAINING UNIT POSITION Position Summary The Home
Care and Hospice RN Clinical Liaison (Union S-RN) is responsible
for modeling the Compassus values of Compassion, Integrity,

foundation. S/he is responsible for upholding the Code of Ethical
Conduct and for promoting positive working relationships within the
company, among all departments, and all external stakeholders. The
Home Care and Hospice RN Clinical Liaison (Union S-RN) assesses
patients referred to Hospice care as well as facilitates discharge
planning from the inpatient setting for hospice and home care
patients, coordinating care with healthcare partners, facilities,
patients, and their loved ones as appropriate. This position is
responsible for hospital and medical facility-based outreach to
identify new referral sources and maintain on-going relationships
that will increase census and preference for hospice and home care
services. Liaisons will use their clinical knowledge, empathetic
communication skills, and marketing abilities to initiate
one-on-one dialog with referral sources, families, and prospective
patients to help explain and educate on hospice and home care
services. This person is a hospice and home care expert and serves
as a resource to their referral sources to ensure extraordinary
patient and customer service. Position Specific Responsibilities
Acts as a hospice and home care expert, serving as a resource to
referral sources, patients, families, and the community. Knowledge
of hospice and home care services necessary to obtain and interpret
information in terms of the patient’s needs to appropriately
receive referrals. Knowledge of Medicare/Medicaid rules/regulations
necessary to understand criteria for determining patient
eligibility for admission to hospice and home care. Provides
ongoing education regarding hospice and home care services to
referral sources, patients, families, and the community.
Communicates with Intake Department, clinical managers, and hospice
center associates regarding availability of resources, bed
availability and referral capacity. Clinically assesses patients
referred to hospice care as well as facilitates discharge planning
from the inpatient setting while also coordinating care and
discharge planning with healthcare partners. Connect with home care
patients prior to discharge from in patient setting to confirm
demographics and desire for home care. Rounds daily in ICU and on
patients at assigned facilities who have been referred to hospice
assessing and evaluating for needs. Communicating with the hospice
clinical team and providers with changes and discharge planning.
Demonstrates strong clinical and critical thinking skills to assess
the hospice patient to assure hospice criteria is met and
communicate effectively to hospice providers. Demonstrates strong
clinical assessment skills to evaluate for the appropriate level of
care for the hospice patient. Assess and provides GIP level of care
as assigned when patients cannot be transferred to hospice.
Provides direct patient care as assigned. Assures all documentation
required by the state, CMS, Mercy Home Care and New Life Hospice
are included in the electronic medical record. Maintain current
knowledge of home care and hospice regulations, industry trends and
reimbursement. Demonstrates the knowledge and skills necessary to
provide care appropriate to the age of the patients served on his
or her assigned unit. Demonstrates knowledge of the principles of
growth and development of the life span and possesses the ability
to assess data reflective of the patient's status and interprets
the appropriate information needed to identify each patient's
requirements relative to his or her age, specific needs and to
provide the care needed as described in departmental policies and
procedures. Performs other duties as assigned. Education and/or
Experience Two (2) year Associates Degree in Nursing required. Four
(4) year Bachelor's Degree in Nursing preferred. Two (2) years of
recent nursing experience in home care or hospice. Previous
experience as RN liaison preferred. Skills Mathematical Skills:
Ability to add, subtract, multiply, and divide in all units of
measure, using whole numbers, common fractions, and decimals.
Ability to compute rate, ratio, and percentage. Language Skills:
Ability to read, analyze, and interpret general business
periodicals, professional journals, technical procedures, or
governmental regulations. Ability to write reports, business
correspondence, and procedure manuals. Ability to effectively
present information and respond to questions from leaders, team
members, investors, and external parties. Strong written and verbal
communications. Other Skills and Abilities: Ability to understand,
read, write, and speak English. Articulates and embraces integrated
healthcare at home philosophy. Other Skills and Abilities:

autonomously, excellent interpersonal skills. Flexible, adaptable

transportation to and from all work-related assignments and
activities. Must be flexible to travel as needed between different
locations, as assigned, in all weather conditions. Other Skills and

Environment: The demands of this role necessitate a team member to
effectively perform essential functions. Adaptations can be made to
accommodate team members with disabilities. Regular standing,
walking, and manual dexterity are fundamental, along with the
ability to lift and move objects up to 50 pounds. Visual acuity
requirements include close and distance vision, color and
peripheral vision, depth perception, and the ability to adjust
focus. In a healthcare setting, exposure to bodily fluids,
infectious diseases, and conditions typical to the field is
expected. Routine use of standard medical equipment and tools
associated with clinical care is essential. This description
