What You Will Do As an RN Case Manager , you’ll provide compassionate, skilled hospice care, including: Performing physical and psychosocial assessments Identifying hospice diagnoses and documenting eligibility Delivering skilled nursing care Managing pain and symptoms Ensuring comfort and quality of life Educating patients and families about end-of-life processes Developing and coordinating individualized care plans Providing emotional and palliative support Supervising hospice aides per Medicare requirements Qualifications What We’re Looking For Active PA Registered Nurse (RN) license in good standing Hospice experience strongly preferred. Strong communication and critical-thinking skills Willingness to travel within Washington/Fayette Counties Valid PA driver’s license, personal vehicle, and insurance Ability to lift/move 50 lbs and transfer patient weight of 100 lbs Ability to perform physical job functions (walking, bending, standing, etc.) Reasonable accommodations may be made to enable individuals with disabilities to perform essential duties without compromising patient care.
