divided into geographical teams to provide empathetic and
professional care to each resident in the place we call home. This
position will support our East2 team working within upper Summit
County, lower Cuyahoga County and lower Geauga County. In this
role, you will facilitate high quality nursing care to patients in
the comfort of their own homes. You will provide assessments,
complex nursing treatments and education to help them avoid
readmissions. This position offers a unique opportunity to meet
patients in a familiar setting where they feel safe with being
open, vulnerable and receptive. A caregiver in this position work
10:00am to 10:30pm with no weekend or holiday requirements. A
caregiver who excels in this role will: - Provide direct nursing
care in accordance with established policies, procedures and
protocols of the agency, taking visits on the uncovered list, late
admissions and other patient visits as needed. - Participate in and
support quality improvement projects. - Participate in departments
and committees as assigned. - Supervise starts of care, resumptions
of care and routine visits as requested. - Perform accurate and
complete admission assessments and reassessments within a specified
time frame. - Assess patient for safety risks, abuse and neglect. -
Serve as a primary coordinator of all disciplines for
well-coordinated patient care. - Assess and document patients' pain
using appropriate pain scale and manage patients' pain then. -
Prioritize and document patient problems based on assessed needs
and problems in collaboration with patients, family and healthcare
team. - Maintain complete and up-to-date documentation and files at
all times regarding patient condition and effectiveness and
timeliness of care and interventions. - Escalate change in patient
condition to patient’s physician as appropriate. Minimum
qualifications for the ideal future caregiver include: - Graduate
from an accredited School of Professional Nursing - Current state
licensure as a Registered Nurse (RN) - Basic Life Support (BLS)
certification through the American Heart Association (AHA) or
American Red Cross - One year of current clinical experience as an
RN - One year of experience in an acute medical/surgical or
homecare setting - Valid Driver’s License (VDL) and proof of
$100,000/$300,000 automobile insurance coverage Preferred

Requirements: - Requires full range of motion, manual and finger
dexterity and eye-hand coordination. - Requires standing and
walking for extensive periods of time. - Requires corrected hearing
and vision to normal range. - Requires some exposure to
communicable disease or bodily fluids. - Medium Work - Exerting 20
to 50 pounds of force occasionally, and/or 10 to 25 pounds of force
frequently, and/or greater than negligible up to 10 pounds of force
constantly to move objects. - Physical Demand requirements are in
excess of those for Light Work. - Must be ambulatory and able to
drive to patients' homes, able to climb stairs, pull, push, and
facilitate movement of the patient. - Must be able to read, write
and communicate with patients. Personal Protective Equipment: -
Follows standard precautions using personal protective equipment as
required. Pay Range Minimum hourly: $33.50 Maximum hourly: $53.60
The pay range displayed on this job posting reflects the
anticipated range for new hires. A successful candidate’s actual
compensation will be determined after taking factors into
consideration such as the candidate’s work history, experience,
skill set and education. The pay range displayed does not include
