for a travel nursing job in Lorain, Ohio. Job Description &

Health Hospice travel contract opportunity supporting patients in
Lorain, OH. Important Requirements: - RN needs to have RN Admission
experience - HCHB charting required Pay: - $2,246.00 (Travel pay
package) - Local pay package available - Mileage reimbursement for
patient related travel Work Schedule: - Mid-shift, either 10:30am –
7:00pm or 11:00am – 7:30pm (Monday to Friday) - 40 hours per week -
Territory: Lorain, Huron, and Erie Contract Details: - 13 week
contract (possible extension) - Nurses are paid by the hour, not by
the visit Job Details - Perform comprehensive assessments and
develop individualized hospice care plans - Deliver skilled nursing
care, symptom management, and emotional support - Educate patients
and families on hospice philosophy and comfort care practices -
Collaborate with the interdisciplinary team to review and adjust
care plans - Maintain accurate documentation in compliance with
hospice standards Qualifications - Current RN License - Must have
at least 12 months of home health hospice experience within the
past 3 years. - OASIS experience - Hands-on HCHB experience
required - Current driver's license - American Heart Association
