Job Description:
The RN Case Manager works in patient homes in the community,
seeing 4-6 patients daily depending on visit type. The RN Case
Manager assesses, oversees, educates, and administers treatments
for patients based on diagnosis and acuity under the supervision of
a clinical Director of Nursing and direction of the ordering
Physician. RN Case Managers create thorough Plans of Care based on
individual patient needs, and always strive to maintain patient
safety, avoiding unecessary hospital admissions.The RN Case Manager
recognizes the need for interventions and communicates on change in
patient status or other events with care team, including case
managers, physicians, family members, home health aides,
therapists, or others. The RN Case Manager has daily communication
with the Director of Nursing, and stops into the office once per
day (or has phone conference with DON if not able to).The RN Case
Manager follows documentation timelines and completes documentation
on EMR software, and also fulfills Electronic Visit Verification
requirements as needed. The RN Case Manager should be able to send
emails, operate laptop computers and various software including
Microsoft Office, utilize the internet, copy machines, fax machine,
scanner, Adobe, phone systems, and others.The RN Case Manager is
part of a team of nurses that rotates a week of on-call every 5-7
weeks. On call responsibility for our single-site, family-owned
agency is generally low volume with the majority of on-call weeks
requiring assistance via telephone and nothing in the field.
On-call duty is paid on top of the agreed upon salary, even if no
calls are received during your week. If a field visit is performed,
additional payment is added on top.
