Responsibilities include but are not limited to:Leading clinical pharmacy programs and services including but not limited to Pharmacy & Therapeutics Committee, Pain stewardship, Hospital Medicine Team meetingsDeveloping drug policy to support optimal pharmaceutical care by assessing, developing, coordinate, implementing, and maintaining clinical pharmacy programs and services.Developing and implementing an Annual Clinical Plan with the Director of Pharmacy and the Regional Vice President of Clinical Service.Provide a leadership role in the development and maintenance of drug policy that promotes optimal cost-effective pharmaceutical care for all patients.Contribute to development and maintenance of clinically relevant tools to support clinical programs and aid the Director in maintaining the pharmacy.Schedule: This position is scheduled to work 40 hours a week primarily daytime hours. The position may be required to work some extended hours which may include some evenings and weekends as business necessitates.Starting Pay: $145,600 - $170,000 annually, (Depending on related work experience) + Bonus eligibility.About the Facility: Memorial Health System is a not-for-profit integrated health system that includes a 200-bed acute care hospital, 25 bed critical access hospital, outpatient infusion center, and two stand-alone ERs. This position is an employee of CPS Solutions, LLC.About the Pharmacy: The Pharmacy is open 24/7.Required Qualifications:Bachelor of Science in Pharmacy OR Doctor of Pharmacy (Pharm.D.) from an ACPE accredited school of pharmacyOne year of clinical hospital pharmacist experienceClinical management experience within a Health SystemOhio licensure in good standing (or Ohio Licensure eligible within 90 days)Experience using computer applications including word processing, spreadsheets and pharmacy information systemsEffective communications skills; verbal and writtenPreferred Qualifications:Residency OR Board Certification OR;3+ years of clinical hospital experience.Hospital Requirements: (may be required)TB GOLD test skin test not accepted(MMR) Measles, Mumps and Rubella or A Blood Titer proving immunityVaricella - (2) documented doses or A Blood Titer provingHep B3 Series (or declination)(Flu) Influenza-required for hire between Oct 1st-April 30thCOVID Vaccine-FullRespirator FIT Mask TestBenefits: All employees enjoy a competitive salary, eligibility to participate in the 401(k) savings plan with a match.
